I need to create a PHP contact form. The only thing is that it has to have attachments (rar and zip files only).

The form should look like this:

Name: [textbox]
Email: [text box and this is where email will come from]
Enquiry: [drop down] Add-on Submission|Distribution Enquiry|License Purchase|Script Installation|Showcase Submission|Other
Attachment: [file]
Comments: [textarea]
Captcha: (If possible)
